Why One Of The Most Vaccinated Places In The U.S. Now Has A COVID-19 Spike

Provincetown, Mass., has one of the highest vaccination rates in the country ?
so why did it just institute an indoor mask mandate after a big spike in
positive COVID-19 tests?

ARI SHAPIRO, HOST:

A town with one of the highest vaccination rates in the country just instituted
an indoor mask mandate after a big spike in positive coronavirus tests.
Jennette Barnes of member station CAI brings us the story from Provincetown,
Mass.

JENNETTE BARNES, BYLINE: For two weeks before the Fourth of July, Provincetown
was averaging zero new cases of coronavirus a day. But not long after that,
Mark Louque, who's from there, came down with symptoms and then tested
positive.

MARK LOUQUE: I've certainly been in touch with a lot of the folks that have
been in and out of Provincetown over the last two weeks, and a large number of
them, a shocking number of them are positive for COVID-19.

BARNES: More than 550 people have tested positive since the Fourth of July.
This in a town where so many people have claimed residency since the pandemic
that the vaccination rate is 116% of its latest census count. Like most of the
people testing positive, Louque is vaccinated, and his so-called breakthrough
infection was pretty mild.

LOUQUE: Yeah, that congestion is just still lingering, but I have pretty much
all of my energy back. I had lost my taste and smell, which have come back. And
I feel like I'm one of the lucky ones because I'm vaccinated.

BARNES: Health experts say that no vaccine is 100% effective at preventing
infections. The coronavirus vaccines are up to 95% effective in preventing
COVID-19 disease, and they're very good at keeping people who do test positive
from getting seriously ill or needing hospitalization. Sean O?Brien, health
director for Cape Cod's Barnstable County, says this summer's outbreak is
following a pattern the county has seen with holidays before, and infections
typically peak a few weeks later.

SEAN O'BRIEN: Unless we are going into a major cycle, like we did last fall and
then again in the winter, you're going to see these kind of ebbs and flows -
for lack of a better term - these peaks and valleys, but very minimal.

BARNES: The outbreak has drawn lots of media attention. Town manager Alex Morse
says there's an upside. Even more people are seeking out vaccination, including
foreign students working here for the summer on J-1 visas.

ALEX MORSE: We've had a lot of interest from businesses in town reaching out -
how can my employees get vaccinated? - business owners getting calls and
finally being able to convince employees that, all right, it's time to - it's
time for me to get vaccinated. Those are hopeful signs.

BARNES: The publicity has business owners like Michael Meyers worried, though.
He owns a clothing store called Wardrobe on Provincetown's main drag.

MICHAEL MEYERS: I'm finding it really discouraging and frightening to see
what's going to come out of the rest of the summer because we were really
counting on this summer, being unemployed throughout the winter and not getting
any income.

BARNES: On a recent weekday, there were still plenty of tourists on the street
where Meyers' store is. Among them, Nikita Alexander and her family.

NIKITA ALEXANDER: We are visiting from Akron, Ohio.

BARNES: They heard about the uptick in cases.

ALEXANDER: And we actually almost did not come today because of that, which is
why we have our mask on on the street. We just wanted to be very careful. And
it worries us that the new strain that's coming out is stronger than the
previous strain.

BARNES: Almost any town that relies on entertaining lots of visitors could see
an outbreak like the one here. Some nightclubs are requiring proof of
vaccination, and at least one resort has pledged to test employees every week.
Public health officials are hoping now that the reality is clear, people will
heed the indoor mask mandate and take other precautions to bring Provincetown's
infection rate back down.

"This finding is concerning and was a pivotal discovery leading to CDC's
updated mask recommendation," CDC Director Dr. Rochelle Walensky said in a
statement. "The masking recommendation was updated to ensure the vaccinated
public would not unknowingly transmit virus to others, including their
unvaccinated or immunocompromised loved ones."

On Tuesday, the CDC reversed course on its prior guidance and recommended fully
vaccinated Americans who live in areas with high Covid infection rates resume
wearing face masks indoors. The guidelines cover about two-thirds of the U.S.
population, according to a CNBC analysis.

While the delta variant continues to hit unvaccinated people the hardest, some
vaccinated people could be carrying higher levels of the virus than previously
understood and are potentially transmitting it to others, Walensky told
reporters on a call Tuesday. She added the variant behaves "uniquely
differently from past strains of the virus."

A CDC document that was reviewed by CNBC warned that the delta variant sweeping
across the country is as contagious as chickenpox, has a longer transmission
window than the original Covid strain and may make older people sicker, even if
they've been fully vaccinated.

Delta, now in at least 132 countries and already the dominant form of the
disease in the United States, is more transmissible than the common cold, the
1918 Spanish flu, smallpox, Ebola, MERS and SARS, according to the document.
Only measles appears to spread faster than the variant.

The data published Friday was based on 469 cases of Covid associated with
multiple summer events and large public gatherings held in July in Barnstable
County, Massachusetts, which encompasses Cape Cod and is just outside Martha's
Vineyard. The events were held in Provincetown, according to NBC News.
Approximately three-quarters, or 74%, of the cases occurred in fully vaccinated
people who had completed a two-dose course of the mRNA vaccines or received a
single shot of Johnson & Johnson's.

Overall, 274 vaccinated patients with a breakthrough infection were
symptomatic, according to the CDC. The most common side effects were cough,
headache, sore throat, muscle pain and fever. Among five Covid patients who
were hospitalized, four were fully vaccinated, according to the agency. No
deaths were reported.

Testing identified the delta variant in 90% of specimens from 133 patients.

While numerous studies have shown that the vaccines don't work as well against
the delta variant as they did against other strains, health officials say they
are still highly effective, especially in protecting against severe illness and
death. Roughly 97% of new hospitalizations and 99.5% of deaths in the U.S. are
among unvaccinated individuals, U.S. health officials repeated this week.

The CDC also said the data has limitations. The agency noted that as
population-level vaccination coverage increases, vaccinated persons are likely
to represent a larger proportion of Covid cases. Additionally, asymptomatic
breakthrough infections might be underrepresented because of detection bias,
the agency said.

The CDC also said the report is "insufficient" to draw conclusions about the
effectiveness of the authorized vaccines against Covid, including the delta
variant, during this outbreak.

Social media posts are misinterpreting the results of a Centers for Disease
Control and Prevention report, which found 74% of people in a COVID-19 outbreak
were vaccinated, to argue against immunization. But experts say the
headline-grabbing statistic is misleading without more context — and doesn’t
mean that the vaccines don’t work.

A statistic from a CDC report about a COVID-19 outbreak in July is fueling false
and misleading claims on social media about how well the Food and Drug
Administration-authorized vaccines protect against the disease, which is caused
by the novel coronavirus.

Clinical trials and studies of the vaccines under real-world conditions show
that the immunizations are highly effective in preventing severe disease and
death, even against the highly contagious delta variant — although they may work
less well against infection and symptomatic disease with delta, as we’ve
written. The delta variant now accounts for around 93% of COVID-19 cases in the
U.S., according to estimates from the CDC.

The statistic in question comes from a paper published on July 30 in the
agency’s Morbidity and Mortality Weekly Report, which documents an outbreak of
COVID-19 in Barnstable County, Massachusetts — elsewhere specified as
Provincetown — that primarily occurred in vaccinated people, following large
public events in the first half of the month.

According to the report, of the 469 people included in the study who were in the
area between July 3 and July 17 and tested positive for the coronavirus, 74%
were fully vaccinated. A total of five people were hospitalized, four of them
vaccinated, and there were no deaths. 90% of the subset of people who had
sequencing performed on their samples were infected with the delta variant.

Although not mentioned in the report, the outbreak overlapped with July Fourth
weekend and “Bear Week,” Provincetown’s annual gathering of gay men; 85% of the
identified infections were in males. In the summer, the town’s population swells
to approximately 60,000 people.

The report noted that at the time of diagnostic testing, the amount of virus
appeared to be about the same in vaccinated and unvaccinated people, a finding
that the CDC cited in its decision on July 27 to recommend masks for everyone,
regardless of vaccination status, in indoor public spaces in areas with
“substantial” or “high” transmission.

But the 74% figure, while correct, can be misleading without the proper context,
experts say, because as vaccination rates increase, it’s entirely expected for a
larger and larger proportion of people who are infected to be vaccinated. It
doesn’t mean the vaccines don’t work.

While the delta variant continues to hit unvaccinated people the hardest, some
vaccinated people could be carrying higher levels of the virus than previously
understood and are potentially transmitting it to others, Walensky told
reporters on a call Tuesday. She added the variant behaves "uniquely
differently from past strains of the virus."
A CDC document that was reviewed by CNBC warned that the delta variant sweeping
across the country is as contagious as chickenpox, has a longer transmission
window than the original Covid strain and may make older people sicker, even if
they've been fully vaccinated.

Delta, now in at least 132 countries and already the dominant form of the
disease in the United States, is more transmissible than the common cold, the
1918 Spanish flu, smallpox, Ebola, MERS and SARS, according to the document.
Only measles appears to spread faster than the variant.
The data published Friday was based on 469 cases of Covid associated with
multiple summer events and large public gatherings held in July in Barnstable
County, Massachusetts, which encompasses Cape Cod and is just outside Martha's
Vineyard. The events were held in Provincetown, according to NBC News.
Approximately three-quarters, or 74%, of the cases occurred in fully vaccinated
people who had completed a two-dose course of the mRNA vaccines or received a
single shot of Johnson & Johnson's.
Overall, 274 vaccinated patients with a breakthrough infection were
symptomatic, according to the CDC. The most common side effects were cough,
headache, sore throat, muscle pain and fever. Among five Covid patients who
were hospitalized, four were fully vaccinated, according to the agency. No
deaths were reported.
Testing identified the delta variant in 90% of specimens from 133 patients.
While numerous studies have shown that the vaccines don't work as well against
the delta variant as they did against other strains, health officials say they
are still highly effective, especially in protecting against severe illness and
death. Roughly 97% of new hospitalizations and 99.5% of deaths in the U.S. are
among unvaccinated individuals, U.S. health officials repeated this week.
The CDC also said the data has limitations. The agency noted that as
population-level vaccination coverage increases, vaccinated persons are likely
to represent a larger proportion of Covid cases. Additionally, asymptomatic
breakthrough infections might be underrepresented because of detection bias,
the agency said.
The CDC also said the report is "insufficient" to draw conclusions about the
effectiveness of the authorized vaccines against Covid, including the delta
variant, during this outbreak.

4,493 Fully Vaccinated Americans Have Died Of COVID-19: CDC

KEY POINTS

More than 4,000 people in the United States have died of COVID-19 despite being
fully vaccinated against the virus, the Centers for Disease Control and
Prevention said in the latest data.

As of Sept. 20, at least 4,493 fully vaccinated Americans died of COVID-19
since the beginning of the pandemic, the CDC stated in the data.

Of the 4,493 deaths, 86% were people aged 65 and older, 44% were females and
19% were patients who did not show any symptoms of COVID-19 or whose deaths
were not coronavirus-related.

According to the data, over the course of the COVID-19 pandemic, 14,643 fully
vaccinated individuals have been hospitalized in the country. At least 69% of
them were people aged 65 and older, 48% were females, and 20% were patients who
had asymptomatic cases of the novel coronavirus or whose admission was not
COVID-related.

The data did not specify how many of the deaths and hospitalizations occurred
in people with underlying medical conditions.

The CDC's data comes as the United States' new COVID-19 vaccinations continue
to lag. The country's seven-day average of people getting their first shot of a
COVID-19 vaccine is 231,695. The figure is 31% less than the numbers reported
over the past week, and 47% less than those recorded in August.

In April, there were more than 4 million doses administered on average,
according to CNN's analysis of the CDC data on vaccination.

As of Tuesday, only 55.8% of the country's total population is fully vaccinated
against COVID-19 and only 64.4% of Americans have received their first doses.

Currently, COVID-19 vaccines are only approved for Americans aged 12 and older.
However, drugmakers Pfizer and BioNTech are expected to request emergency use
authorization for a COVID-19 vaccine for children ages 5 to 11.

"A formal submission to request Emergency Use Authorization (EUA) of the
companies' COVID-19 vaccine in children 5 to <12 years of age is expected to
follow in the coming weeks. Submissions to the European Medicines Agency (EMA)
and other regulatory authorities are also planned," the drugmaker said in a
press release.

Until vaccines are approved for younger children, the CDC is urging students,
teachers and school staff members to wear masks and observe social distancing
to prevent COVID-19 infection.

COVID-19 has so far infected 44,054,825 people and claimed 711,222 lives in the
United States, according to worldometer data.
